  • TAX IDENTIFICATION NO. (TIN) is a mandatory requirements for the CAIF.

FOREIGNER:

  •  
  • Please provide us with a photocopy of two (2) current and official Identification Cards (ID) by your local government, to wit;
    1. Driver’s License (unexpired)
    2. Valid passport (unexpired)

  • Please sign the Photocopy of your Identification Card (ID) for authentication.

  • Secretary's Certificate Authorizing the corporation to open an account with LUCKY SECURITIES INC. and enumerated list of authorized Signatory with corresponding signature.
  • Xerox Copy of Two (2) Valid Identification Card (ID) of Authorized Signatories issued by Local Government (Both Primary or 1 Primary and 1 Secondary). Please sign the photocopy of the identification Card (ID) for authentication.
  1. PRIMARY : (Drivers License, Passport, SSS, Pag-Ibig, TIN, GSIS, COMELEC)
  2. SECONDARY: (Company ID, Postal, Senior Citizen, Barangay ID)
  • Latest Financial Statement
  • TAX IDENTIFICATION NO. (TIN) is a mandatory requirements for the CAIF.

  • Articles of Incorporation and By-Laws duty Authenticated by Philippine  Consul.
  • Secretary's Certificate Authorizing the corporation to open an account with LUCKY SECURITIES, INC. and enumerated list of authorized Signatory with corresponding signature.
  • Xerox Copy of Two (2) Valid Identification Cards of Authorized Signatories recently (Driver's License, Passport and Any Government Issued Identification Card). Please sign the Photocopy of the Identification Card (ID) for Authentication.
