Lucky Securities, Inc.

Individual Account

  1. State your intent to open an account and email the following requirements to egluckysec@gmail.com or you may personally submit them at the Lucky Securities office at Unit 1106, PSE Tower, 5th Avenue cor. 28th Street, Bonifacio Global City, Taguig City:
      • Photocopy of two (2) current and official Identification Card (ID). The following are accepted:
        • Driver’s License (unexpired)
        • Valid passport (unexpired)
        • Company ID
        • SSS/GSIS
        • Bank ATM / Passbook
      • Please sign the photocopy of your Identification Card (ID) for authentication.
      • Individual CAIF should be filled out for AND / OR accounts
      • TAX IDENTIFICATION NO. (TIN) is a mandatory requirement for the CAIF.

    • If Foreign:
      • Photocopy of two (2) current and official Identification Card (ID) by your local government. The following are accepted:
        • Driver’s License (unexpired)
        • Valid passport (unexpired)
        • Company ID
      • Please sign the photocopy of your Identification Card (ID) for authentication.
      • Individual CAIF should be filled out for AND / OR accounts
      • Bank Certification (Account with Bank)

  2. Completely fill-out the following forms:
    • Customer Account Information Form (CAIF)
    • Online Trading Service Agreement
    • Lucky Securities Data Protection Notice
    • FATCA Due Diligence Form
    • Lucky Securities Specimen Card

  3. Submit the filled-out forms and complete requirements via email to egluckysec@gmail.com or you may personally submit them at the Lucky Securities office at Unit 1106, PSE Tower, 5th Avenue cor. 28th Street, Bonifacio Global City, Taguig City.

  4. Kindly expect feedback within 1-2 trading days. Confirmation on the successful creation of your account will be sent via email.

Corporate Account

  1. State your intent to open an account and email the following requirements to egluckysec@gmail.com or you may personally submit them at the Lucky Securities office at Unit 1106, PSE Tower, 5th Avenue cor. 28th Street, Bonifacio Global City, Taguig City:
      • Certified True Copy of Articles of Incorporation and By-Laws
      • Secretary’s Certificate authorizing the corporation to open an account with Lucky Securities, Inc. and enumerated list of authorized signatories with corresponding signature
      • Photocopy of Two (2) Valid Identification Card (ID) of Authorized Signatories issued by local Government (both Primary or 1 Primary and 1 Secondary). Please sign the photocopy of the identification card for authentication.
        • Primary: Driver’s License, Passport, SSS, Pag-ibig, TIN, GSIS, Comelec
        • Secondary: Company ID; Postal ID; Senior Citizen ID, Barangay ID
      • Latest Financial Statement
      • TAX IDENTIFICATION NO. (TIN) is a mandatory requirement for the CAIF.

    • If Foreign:
      • Articles of Incorporation and By-Laws duly Authenticated by Philippine Consul.
      • Secretary’s Certificate authorizing the corporation to open an account with Lucky Securities, Inc. and enumerated list of authorized signatories with corresponding signature
      • Photocopy of Two (2) Valid Identification Card (ID) of Authorized Signatories issued by local Government (Driver’s License, Passport, any government-issued Identification Card). Please sign the photocopy of the identification card for authentication.
      • Bank Certificate (account with Bank)
      • Latest Financial Statement

  2. Completely fill-out the following forms:
    • Customer Account Information Form (CAIF)
    • Online Trading Service Agreement
    • Lucky Securities Data Protection Notice
    • FATCA Due Diligence Form
    • Lucky Securities Specimen Card

Yu & Company, Inc.

Direct Market Access Account (DMA)

      1. Download and fill out this form.

      2. Submit the filled-out form along with the following to yunco@gmail.com:
        • 1x1 ID picture,
        • Photocopy of any two (2) government valid ID’s containing photo and signature. We accept:
          • UNIFIED MULTI-PURPOSE ID
          • NATIONAL ID
          • PASSPORT ID
          • TIN CARD
          • SSS ID
          • PHILHEALTH ID
          • PAG-IBIG ID
          • SENIOR CITIZEN ID
          • DRIVER'S LICENSE
          • VOTER'S ID
          • PRC ID
        • If FOREIGN account: Alien certificate of registration identity card with permanent resident status (if the status on the ACR I-CARD is non-immigrant, the expiry date of the ACR I- card should be more than one year from booking date)
        • Note: There is no minimum deposit.

      3. Kindly expect feedback within 1-2 trading days. Once approved, you will receive email instructions to activate your account.

Broker-Assisted Account

      1. Download and fill out this form.

      2. Submit the filled-out form along with the following to cs.yunco@gmail.com:
            • 1x1 ID picture,
            • Photocopy of any two (2) government valid ID’s containing photo and signature. We accept:
              • UNIFIED MULTI-PURPOSE ID
              • NATIONAL ID
              • PASSPORT ID
              • TIN CARD
              • SSS ID
              • PHILHEALTH ID
              • PAG-IBIG ID
              • SENIOR CITIZEN ID
              • DRIVER'S LICENSE
              • VOTER'S ID
              • PRC ID

            • If FOREIGN account: Alien certificate of registration identity card with permanent resident status (if the status on the ACR I-CARD is non-immigrant, the expiry date of the ACR I- card should be more than one year from booking date)
            • Note: There is no minimum deposit.
            • Minimum Deposit for Corporate Accounts: 1,000,000

Eagle Equities, Inc.

Individual Account

  1. Visit Eagle Equities’ office at 779 Harvard St. Wack-Wack Village, Mandaluyong City to submit the following requirements:
      • Two (2) valid identification cards recently issued by local government (photocopy both primary or 1 primary and 1 secondary)
        • Primary: Driver’s License; Passport; SSS; Pag-ibig; TIN; GSIS, Comelec
        • Secondary: Company ID; Postal ID; Senior Citizen ID, Barangay ID 
    • If Foreign:
      • Two (2) valid identification cards recently issued by local government.
        • Driver’s license; passport and any government-issued identification card
      • Billing statement wherein address is the same as indicated in CAIF.
      • Utilities billing, communication billing, credit card billing.

      • Bank certification stating that such person has an existing account.

  2. Completely fill out the following account opening forms:

    • Customer Account Information Form (CAIF)
    • Online Trading Service Agreement
    • Eagle Equities Data Protection Notice
    • FATCA Questionnaire
    • Eagle Equities Signature Card

Corporate Account

  1. Visit Eagle Equities’ office at 779 Harvard St. Wack-Wack Village, Mandaluyong City to submit the following requirements:

      • Two (2) valid identification cards recently issued by local government (photocopy both primary or 1 primary and 1 secondary)
        • Primary: Driver’s license; passport; SSS; Pag-ibig; TIN; GSIS, Comelec
        • Secondary: Company ID; postal ID; senior citizen, barangay
        • Latest audited financial statement

    • If Foreign:
      • Certified true copy of articles of incorporation and by-laws.
      • Secretary’s certificate authorizing the corporation to open an account with Eagle Equities, Inc. and enumerating list of authorized signatory/(ies) with corresponding signature of signatory/(ies).
      • Two (2) valid identification cards recently issued by local government.
        • Driver’s license; passport and any government-issued identification card
      • Latest audited financial statement.
      • Foreign corporation, above documents must be authenticated by the Philippine Embassy (Consularized) or apostilled issued in accordance with the Hague convention whichever is applicable.

  2. Completely fill out the following account opening forms:
    • Customer Account Information Form (CAIF)
    • Online Trading Service Agreement
    • Eagle Equities Data Protection Notice
    • FATCA Questionnaire
    • Eagle Equities Signature Card
    • Once downloaded, fill out and email to your respective brokers.
